What this site is
This website is an informational profile for Zobia Zafar, MPAS, PA-C. It describes her training, credentials and the care she provides at Optima Medical in Laveen, Arizona. It is not a patient portal and it is not a place to send medical information.
Information this site collects
There are no contact forms on this website, so it does not ask you for your name, your email address or any health information. Like almost every website, the web server keeps standard technical logs, which can include your IP address, browser type and the pages you requested. Those logs are used for security and to keep the site running.
Appointment booking happens elsewhere
Booking links on this site take you to third-party services, Zocdoc and Curogram, and to Optima Medical. Anything you enter on those services is handled under their own privacy policies and terms, not this one. Please read theirs before entering personal or health information.
Medical records and HIPAA
Your medical records are held by Optima Medical as your treating practice, and are governed by their notice of privacy practices and by HIPAA. This website holds no medical records and no protected health information.
Cookies and analytics
No analytics or advertising trackers are installed on this site at the time of writing. If that changes, this section will be updated to say what is running and what it collects.
